What is A Wave?
Cannon A waves are big waves that can be noticed in the neck veins during a physical check-up. They occur when the two chambers of the heart (the atria and the ventricle) contract at the same time, causing a spike in the pressure in the right atrium. These waves usually occur irregularly and sporadically. They are found in patients with heart conduction issues or specific heart rhythm disorders. Cannon A waves can also be detected on an EKG, which is a test that checks the heart’s electrical activity.
What Causes A Wave?
Cannon A waves are associated with irregular heart rhythms that disrupt the typical flow of blood in the heart, leading to high pressure in the jugular vein, a vein in the neck. Various heart rhythm disorders can lead to these waves. For example, a blocked heart signal, specifically total blockage, can lead to Cannon A waves. These waves might also be seen in fast-paced heart rhythm disorders due to the disconnection between two crucial parts of the heart. Another cause is Pacemaker syndrome, which occurs when the two central chambers of the heart aren’t properly synchronized.
Cannon A waves must not be confused with giant A waves, which are associated with changes in the structure of the right heart such as issues with the tricuspid valve, right heart enlargement, and high blood pressure in the lung arteries. To someone examining the jugular vein, giant A waves and Cannon A waves might look the same. It could be challenging to tell the difference between them just by physical exams alone.
Risk Factors and Frequency for A Wave
Jugular vein pulsation, or JVP, is useful because it gives us valuable data about the central venous pressure, also known as CVP. JVP is a reliable tool that helps doctors identify different heart problems that can change the CVP. The Canon A wave is a feature of JVP that might not be noticed without careful examination, especially as physical exam skills may not be as strongly emphasized in today’s medical practice. Recent research hasn’t clarified how often the Canon A waves occur in cases of irregular heart rhythms.
Signs and Symptoms of A Wave
People with certain heart conditions may feel a pulsing sensation in their neck and abdomen as the pulse wave moves back into their veins. They could also experience symptoms like headaches, coughs, and jaw pain. A notable symptom can be increased urination, which is linked to higher levels of a hormone known as BNP that results from added stress on the heart. This condition is often tied with higher heart pressures, which can result in low blood pressure throughout the body.
The doctor will check for a specific kind of neck vein pulsation while listening to the patient’s heart. It’s important to note that the venous pressure curve is closely related to the sounds made by the heart. A specific wave, known as the ‘A wave,’ is followed closely by a heart sound, marking the closing of the heart valves that regulate blood flow.
The unique pattern of neck vein pulsation seen in patients is often referred to as the “frog sign”. It is worth distinguishing between regular and irregular patterns of these “Cannon A waves”. Regular wave patterns might indicate specific heart rhythms or rapid heart rate conditions. In contrast, irregular wave patterns could point to a disconnected rhythm of the two main areas (atria and ventricles) of the heart or irregular heartbeats.
Testing for A Wave
If a patient experiences symptoms that could be linked to Cannon A waves or if a physical examination shows signs of Cannon A waves, then further tests are needed. Cannon A waves and giant A waves need to be differentiated and for this, an ECG (electrocardiogram) and a heart ultrasound (echocardiography) are recommended.
The ECG is particularly useful in checking for irregular heart rhythms. Specifically, if P waves (a type of wave on the ECG) occur within the QT interval – the time it takes for the heart to contract and then refill with blood – it could suggest the presence of Cannon A waves.
The ultrasound of the heart is used to check for any structural changes. This could include things like an enlargement of the right side of the heart (hypertrophy), issues with the tricuspid valve (the valve that controls blood flow between the right side chambers of the heart), and high blood pressure in the lungs (pulmonary hypertension).

What else can A Wave be?
When a doctor checks a patient’s neck, they may notice different types of pulses. These can suggest several health conditions, such as an excessive amount of fluid in the body, a blockage in the superior vena cava (a major vein in the body), or an increased pressure within the chest. To understand what these pulses mean, the doctor would first determine whether these pulses are coming from arteries or veins based on their location and how strong they are when physically felt. A key point is that pulses in the arteries are usually stronger than those in veins. The right internal jugular vein, a major vein in the neck, is often checked because it aligns directly with the superior vena cava.
The pattern of the pulse in the veins can change in different conditions, creating confusion. For instance, large and prominent waves can be seen in a condition called tricuspid regurgitation. This event is referred to as Lancisi’s sign. A swollen jugular vein could suggest a blockage in one of the lung’s arteries (part of a condition known as Beck’s Triad). If the jugular vein is enlarged but there’s no visible pulse, it might be a sign of a superior vena cava obstruction. This underlines how observing the veins can provide significant insights into the heart’s right side’s function.
